Who would’ve known the drug that reverses unfractionated heparin derives from salmon sperm. By itself, protamine is a basic compound that acts as a weak anticoagulant. When administered in the presence of heparin, protamine will bind to the highly acidic structure, and form a non-active salt compound with no anticoagulant effects. Dosing is based on prior infusion rates. Be mindful that rapid IV push can cause severe hypotension and anaphylactoid-like reactions. Protamine is a true antidote for unfractionated heparin in the setting of life-threatening bleed, but has variable efficacy in reversing low molecular weight heparin.
